Schedule Overview:

Regular hours are typically Monday to Friday, from 8:00 AM to 4:30 PM. There are no on-call, weekend, or holiday obligations.

The role involves managing approximately 6 patients daily.

The service area is located on the north side of Milwaukee, specifically north of I-94, between I-41 and I-43.

Excellent communication skills with supervising physical therapists are essential.

Key Responsibilities: Deliver and advance patient care in accordance with the therapist's assessment and care plan. Treatment modalities include patient education, exercise regimens, gait training, mobility assistance, balance and coordination exercises, application of modalities, soft-tissue/joint mobilization, and training for orthotic/prosthetic devices. Oversee a personal caseload, including scheduling and notifying supervisors of any coverage requirements. Instruct patients and caregivers on appropriate treatment methods and supportive activities, while formulating plans for acquiring necessary adaptive devices. Conduct ongoing evaluations of patient responses to treatments and education. Discuss caseload challenges and changes with the supervisor to develop action plans. Prepare for patient discharge by providing written home care programs, educating patients and families, and coordinating equipment orders under the guidance of the supervising therapist. Maintain accurate clinical and patient documentation in compliance with agency, state, and federal regulations. Explain agency services and programs to patients and their families. Ensure cleanliness and organization of the treatment area by tidying up after patient sessions and storing equipment properly. Adhere to established policies and procedures, while regularly reviewing updates or modifications. Perform various patient positioning and transportation tasks, which may involve lifting and pushing/pulling, utilizing appropriate techniques. Demonstrate the knowledge and skills necessary to provide age-appropriate care for patients. This includes understanding growth and development principles across the lifespan and being able to assess and interpret patient data to meet individual needs. Required Licensure and Certifications: A valid driver's license issued by the Division of Motor Vehicles, and A Physical Therapist Assistant license issued by the state of Wisconsin, and Basic Life Support (BLS) certification for Healthcare Providers from the American Heart Association (AHA) must be obtained within six months unless exempted by the department leader. Educational Qualifications: Associate's Degree in Physical Therapy. Experience Requirements: Generally requires two years of experience in physical therapy assisting within home health care, hospital acute care, or similar environments. Knowledge, Skills & Abilities: Proven proficiency in clinical skills with the capacity to work independently and make informed decisions. Ability to demonstrate knowledge and skills necessary for providing age-appropriate care. Strong interpersonal, organizational, problem-solving, and decision-making skills. Exhibits initiative, adaptability, and positive interpersonal interactions. Intermediate proficiency in computer usage. Physical Demands and Work Environment: Must possess and provide proof of a valid Driver License and have access to a registered, functional vehicle with adequate insurance. Capable of manual dexterity to operate various medical equipment and office tools. Must have the physical ability to navigate confined spaces, including bending, twisting, kneeling, and occasionally reaching overhead. Able to work safely with individuals of varying sizes and weights, with diverse medical conditions and physical limitations. Must be able to: Lift up to 50 lbs from floor to waist. Lift up to 20 lbs from waist to overhead. Lift and carry up to 40 lbs at waist height over reasonable distances. Must be able to: Push/pull with 30 lbs of force. Perform a sliding transfer of 150 lbs with assistance. Capable of sitting, standing, walking, and climbing stairs for extended periods to complete necessary paperwork, access patients' residences, travel for assigned visits, and participate in various work-related activities. Must possess the stamina to attend to patient care needs for three or more hours without a complete break. Operate all equipment necessary for job performance.
