Travel Nurse for Case Management Role in Thousand Oaks, CA

We are seeking a highly skilled Travel Nurse to join our team in Thousand Oaks, CA. As a RN Case Manager, you will play a critical role in advocating for patients and ensuring they receive the best possible care.

Job Responsibilities- Evaluate and assess new patients to develop effective care plans.
- Create and update comprehensive short and long-term patient care plans to ensure seamless transitions.
- Educate families and patients about available resources and healthcare options to promote informed decision-making.
- Collaborate with other RNs, doctors, and social workers to coordinate care and achieve optimal outcomes.
- Properly document and submit medical records to maintain accurate and up-to-date patient information.
- Communicate with insurance companies to navigate coverage and appeal decisions when necessary.
- Analyze results, monitor variances, identify trends, and recommend actions to management to improve financial performance.
To be successful in this role, you should have:
- A BLS certification.
- At least 2 years of recent case management experience.
