Job Summary

We are seeking a dedicated and compassionate Registered Nurse/Licensed Practical Nurse to join our team at Ashton Creek Health and Rehabilitation. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to a positive and supportive work environment.

Responsibilities
  • Provide professional assessments and documentation of patient health and assist with management of assigned nursing units and staff.
  • Participate in the development and implementation of patient care plans, maintaining up-to-date nursing care plans and documenting progress.
  • Make daily and periodic patient visits for monitoring and observing physical/mental condition.
  • Perform routine nursing procedures while working closely with physicians and families to maintain patient health/mental state.
  • Provide emotional support to patients and their families and handle resident and/or visitor complaints, allegations, accidents or incidents that may occur.
  • Provide medical record documentation, complete 24 hour reports, end of shift reports, shift narcotic counts and document the destruction of outdated/discontinued medications.
  • Maintain up-to-date patient medical records.
Requirements
  • Registered Nurse RN/LPN Qualifications
  • Graduate of an accredited school of nursing and possess a valid RN/LPN license in good standing according to State and Federal requirements.
  • Have a thorough understanding of the principles of safe effective nursing practices within the State of Indiana.
  • Basic Computer knowledge and skills.
  • A passion for caring for people.
  • Personal integrity and the ability to deal tactfully with patients, family members, visitors, government agencies and other personnel.
  • The ability to be discrete and protect the integrity of confidential information and stay within Corporate Compliance and HIPAA.
  • Knowledge on the use of personal computers, Word and Excel, and working with office machines such as fax and copying machines.
