Position Overview

Role Summary: The Personal Care Assistant plays a vital role in providing essential support to clients in their homes, ensuring their comfort and well-being.

Key Responsibilities:

  • Engage regularly with clients, their families, and various community members to foster a supportive environment.
  • Under the guidance of a Registered Nurse, deliver personal and palliative care to individuals facing terminal illnesses, encouraging their participation in family life.
  • Assist family members in managing caregiving responsibilities while promoting the client's independence.

Core Duties:

  • Support the client's physical health by adhering to the care plan established by the RN, which includes maintaining personal hygiene, preparing nutritious meals, and assisting with mobility.
  • Provide emotional and psychological support to clients and their families, fostering trust and confidentiality.
  • Document care activities in accordance with company policies and report any changes in the client's condition to the supervising nurse.
  • Collaborate with an interdisciplinary team to ensure comprehensive care, addressing any concerns regarding the client's comfort and safety.
  • Respect and accommodate cultural practices while ensuring the safety and dignity of all clients.

Qualifications:

  • Completion of a state-required background check.
  • Registration on the Nursing Assistant Registry for Minnesota and North Dakota.
  • Current Basic Life Support (BCLS) certification.
  • Possession of a valid driver's license and reliable transportation.
  • Demonstrated maturity and interpersonal skills to build effective relationships with clients and their families.
  • A genuine passion for providing care to individuals with health challenges.
  • Successful completion of orientation and ongoing competency assessments.
  • Ability to perform physical tasks associated with caregiving.
  • Proficiency in English to effectively communicate with clients, families, and healthcare staff.

Reporting Structure: This position reports to the Regional Clinical Manager.
