Key Responsibilities- Assessment and Planning: Collect and analyze patient data to identify actual or potential diagnoses, problems, and issues. Develop individualized care plans that address patient needs and outcomes.
- Care Coordination: Coordinate care delivery with other healthcare professionals, including physicians, therapists, and social workers. Ensure that patient care is delivered in a safe and effective manner.
- Education and Support: Provide education and support to patients and their families to promote health and wellness. Develop and implement patient education plans that address specific patient needs.
- Communication and Collaboration: Communicate effectively with patients, families, and healthcare professionals to ensure that patient care is delivered in a coordinated and effective manner.
- Education: Graduate from an accredited School of Professional Nursing.
- Experience: 1 year of nursing experience in a medical-surgical setting.
- Certifications: BLS required. Valid, active unencumbered Nursing license approved by the Georgia Licensing Board for GA locations and Alabama Licensing Board for AL locations.
This role requires the ability to work in a fast-paced environment and to prioritize tasks effectively. The Nurse Care Coordinator must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. The ability to respond quickly to changes in patient and/or unit conditions is also required.
