Responsibilities:- Provide clinical care and consultation to patients in their homes
- Prescribe necessary treatment within the scope of practice and assess patient clinical problems and healthcare needs
- Educate patients on preventive care and health safety issues
- Evaluate and treat patients in skilled nursing facility settings or post-discharge at residential facilities or private residences
- Perform annual wellness exams for senior Medicare patients at residential facilities or clinic settings
- Cover for other primary care physicians when necessary
- Evaluate and treat patients independently
- Obtain patient medical history and documentation in EMR
- Create progress notes for each encounter with each patient evaluated
- Promote a friendly, warm, and exceptional customer experience
- Secure patient information and maintain patient confidence by completing and safeguarding medical records
- Prescribe pharmaceuticals, medications, and treatment regimens as appropriate to assessed medical conditions
- Master's Degree
- Board certification and license (RN, FNP)
- Active DEA license or ability to obtain
- NPI number and furnishing license
- Outstanding observation and analytical skill to diagnose and treat patients effectively
- Excellent verbal, written, and interpersonal skills
- Fully bilingual in both English and Spanish is a plus
- Compassionate and skilled Nurse Practitioner
- Eager to provide top-notch care to patients at home
- Professional seeking flexible work schedule
- Available day(s) Monday - Saturday
