  • Conduct in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identify diagnoses to inform care management and active medical management in the furtherance of treatment.
  • Develop a list of current and past medical conditions using clinical knowledge and judgment, along with assessment findings.

About Our Organization

Licensure must be unencumbered, free from any open or unresolved disciplinary actions, including probation or restrictions against practicing privileges.

Active national NP or PA certification is required, or the ability to obtain national certification in the state of assignment by start date.

Requirements and Qualifications
  • Able to complete the physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and various dwelling conditions; sit, stand, and kneel to perform physical assessments.
  • Home Health care or home visit experience is necessary.

**Student loan repayments will cease if an employee leaves Optum. To receive the full benefit, the employee must remain in an Advanced Practice Clinician or Physician role within Optum for 36 months.
