Position Overview:

Key Responsibilities:
As a Clinical Case Manager, you will:
1. **Coordinate Patient Care:** Facilitate patient care progression by collaborating with a multidisciplinary team, including physicians and nursing staff, to ensure timely and appropriate care.
2. **Monitor Patient Progress:** Oversee patient progress, intervening as necessary to maintain a high standard of care that is both efficient and cost-effective.
3. **Utilization Management:** Conduct Utilization Management and Quality Screening for assigned patients, applying clinical appropriateness criteria to monitor admissions and continued stays.
4. **Discharge Planning:** Manage all aspects of discharge planning, including assessing patient needs and developing individualized care plans in collaboration with the healthcare team.
5. **Data Management:** Utilize data to inform decisions and implement performance improvement strategies, focusing on clinical outcomes and patient satisfaction.

Qualifications:
- Graduate of an accredited nursing program; a Bachelor’s degree in Nursing is preferred.
- Certification in Case Management is required within two years of hire.
- A minimum of three years of nursing or social work experience in an acute hospital setting or comparable clinical environment.
- Strong analytical and data management skills, with a current understanding of case management and managed care reimbursement.
