Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Nurse Practitioner. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, working closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.

Responsibilities
  • Assess patients' physical and emotional needs, developing and implementing care plans to meet their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring, in accordance with physician orders and individual care plans.
  • Collaborate with patients, families, and healthcare teams to educate and empower patients to manage their conditions and maintain independence.
  • Document patient progress, identifying areas for improvement and making recommendations for care plan adjustments as needed.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in home health care.
Requirements
  • Current Missouri or Compact Registered Nursing or Licensed Practical Nursing License.
  • Strong organizational and self-management skills, with ability to prioritize tasks and manage multiple patients' needs.
  • Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Valid driver's license and reliable transportation.
