Key Responsibilities- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Provide education and support to patients and families on health promotion, disease prevention, and management of chronic conditions.
- Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines.
- Maintain accurate and timely documentation of patient care and outcomes.
- Current RN licensure in the state of Washington.
- Basic Life Support certification.
- Associate's degree in Nursing from an accredited program.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Ambulatory care nursing experience.
- Primary nursing or case management experience.
- Bachelor of Science in Nursing (BSN).
