Job Summary

We are seeking a caring and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will work closely with patients, families, and healthcare professionals to coordinate care and achieve optimal patient outcomes.

Responsibilities

  • Coordinate patient care and services to ensure seamless delivery of care
  • Communicate effectively with patients, families, and healthcare professionals to gather information and coordinate care
  • Develop and implement individualized care plans to meet the unique needs of each patient

Requirements

To be successful in this role, you must have:

  • A minimum of an Associate Degree in Nursing (A.D.N.) from an accredited school of nursing
  • A valid Registered Nurse License in the state of practice
  • Current BLS/CPR certification
  • Previous experience in patient care coordination, preferably in dialysis or a related field

  • An estimated salary range of $80,000 - $105,000 per year
