Job Summary:

Responsibilities:
  • Develop and implement Individual Plans of Care, incorporating strategies to enhance daily living skills, social, emotional, and physical well-being.
  • Administer medication, as certified, and maintain accurate records, adhering to agency policies and regulatory standards.
  • Conduct daily monitoring of medical, physical, and behavioral changes, reporting promptly to supervisors and ensuring client safety.
  • Maintain a clean and safe living environment, including meal preparation, household management, and vehicle upkeep.
  • Transport clients to community-based outings, adhering to health and safety protocols.

Requirements:

  • A high school diploma or GED, with preference for two years of experience in Human Services or a related field.
  • Valid CT driver's license, good driving record, and reliable transportation.
  • Excellent communication and organizational skills, patience, and a nurturing temperament.

Estimated Salary: $45,000 - $55,000 per year, based on experience.
