We are seeking a skilled Physical Therapist to join our team in the Acute Care Rehabilitation department at Advocate Lutheran General Hospital. This is a part-time position with a weekend program schedule, requiring a minimum of 20 hours per weekend.
Key Responsibilities- Interpret physician referrals and conduct comprehensive evaluations to develop individualized treatment plans.
- Implement and monitor treatment plans, adjusting as needed to ensure optimal patient outcomes.
- Collaborate with physicians and other healthcare professionals to ensure seamless patient care.
- Document patient progress and maintain accurate records.
- Participate in performance improvement activities to enhance patient care and outcomes.
- Basic Life Support (BLS) certification from the American Heart Association.
- Physical Therapist license issued by the state of Wisconsin.
- Bachelor's Degree in Physical Therapy.
- Excellent communication and interpersonal skills.
- Intermediate computer skills.
Advocate Aurora Health offers a competitive salary and benefits package, including a $2500 sign-on bonus payable in two installments. Additionally, you will receive an hourly premium of $11/hour.
