Job Summary

We are seeking a highly skilled Social Worker (BSW) - Volunteer Coordinator to join our team. As a key member of our hospice care team, you will provide psychosocial support to patients and their families, while also representing our volunteers and advocating for volunteer services.

Key Responsibilities
  • Conduct psychosocial assessments and provide case management services to patients and families.
  • Provide social work expertise regarding changes to patient care levels.
  • Develop and implement casework and service management plans for patients requiring information about or monitoring of family relationships, cultural and spiritual attitudes, long-term care planning, and advanced care planning.
  • Monitor patient safety and supervise volunteer activities.
  • Assess patient and family requests for volunteer services and assign volunteers as appropriate.
  • Oversee volunteer trainings, maintain volunteer personnel files, and ensure timely documentation.
Requirements
  • Bachelor's Degree in Social Work from a CSWE-accredited school.
  • Licensure as required by the state in which the hospice is located.
  • One year of experience as a Social Worker in a healthcare and/or hospice setting, with three to five years preferred.
  • Knowledge of terminally ill patients and their families, as well as understanding of hospice and psychosocial dynamics of illness, loss, and death.
