Responsibilities
  • Conduct thorough assessments of clients' physical abilities and develop personalized treatment plans to address their needs.
  • Implement evidence-based interventions, including manual therapeutic exercises, gait training, and balance exercises, to promote clients' physical function and mobility.
  • Collaborate with other healthcare professionals, including nurses and occupational therapists, to ensure comprehensive care and coordination.
  • Provide education and training to clients and their families on proper techniques and strategies for maintaining physical function and preventing further injury or decline.
  • Document all client interactions, assessments, and interventions in a timely and accurate manner, using electronic medical records.
Requirements
  • Current licensure as a Physical Therapist in South Carolina.
  • Minimum of one year of recent experience as a Physical Therapist, preferably in home health care.
  • Graduation from an accredited Physical Therapy program.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to work independently and manage time effectively, with a focus on delivering high-quality care and meeting client needs.
