About the Role

We are seeking a skilled Physical Therapist to join our Home Health team in Douglas, Georgia.

This is a unique opportunity to work with patients in their homes and provide personalized care that improves their quality of life.


Responsibilities
  • Evaluate patient needs and develop effective treatment plans to improve mobility, endurance, and overall well-being.
  • Treat patients to relieve pain, restore function, and maintain maximum functioning using evidence-based practices.
  • Collaborate with healthcare teams to ensure seamless care coordination and optimal patient outcomes.

Requirements
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification required.
  • Able to travel to patient locations in a reliable vehicle.
