We are seeking a compassionate Terminal Illness Support Professional to join our team at Mid Michigan Home Health & Hospice. As a key member of our interdisciplinary group, you will provide essential medical social work services to patients and their families, focusing on addressing the physical, emotional, and spiritual aspects of terminal illness.

Key Responsibilities:

This role requires a graduate degree in social work (MSW) from a Council on Social Work Education-accredited program. You will assess patients' psychosocial status, provide counseling and crisis intervention, and educate patients and families about advance care planning and practical support services.

  • Maintain collaborative relationships with healthcare professionals and community agencies.
  • Participate in quality assessment performance improvement teams and activities.
  • Develop and implement individualized plans of care in collaboration with patients and families.

A minimum of one year's experience in healthcare is necessary, with hospice experience preferred. The successful candidate will also be a licensed driver with a valid automobile insurance policy. The salary for this position is estimated to be around $67,000 per annum, based on industry standards and location.
