Job Summary
We are seeking a highly motivated and detail-oriented Licensed Practical Nurse (LPN) to join our team at Riverton Rehabilitation and Healthcare Center. As an LPN, you will play a vital role in providing high-quality patient care, working closely with physicians, therapists, and other healthcare professionals to develop and implement individualized plans of care.

Salary and Benefits
The salary for this role is approximately $45,000 per year, depending on experience, plus opportunities for shift differentials and bonuses. We also offer a comprehensive benefits package, including health, vision, and dental insurance, as well as 401(k) matching and employee assistance programs.

Key Responsibilities
- Deliver patient-centered care, prioritizing the needs and preferences of each resident
- Administer medications and treatments as prescribed by physicians, ensuring accuracy and attention to detail
- Collaborate with the interdisciplinary team to develop and implement individualized plans of care, taking into account resident goals and outcomes
- Maintain accurate and timely documentation of patient care and treatment plans

Requirements and Qualifications
- Current, unencumbered active license to practice as an LPN in Pennsylvania
- Proven ability to work effectively in a fast-paced environment, prioritizing tasks and managing competing demands
- Strong communication and interpersonal skills, with the ability to build rapport with residents, families, and healthcare professionals
