Your Role in Patient Care:

The Practical Nurse (LPN) is essential in patient care, actively participating in the nursing process of assessment, planning, implementation, and evaluation.

Under the guidance of Registered Nurses (RNs), the LPN offers direction and support in patient education and activities that align with their training and demonstrated skills.


Key Responsibilities:

Under RN supervision, evaluate patients' physical, psychological, and social aspects in both healthcare and housing settings, contributing to personalized treatment planning and consulting with RNs and staff as necessary.

Execute treatment plans directed by healthcare practitioners, administer medications (including IV therapy if certified), and conduct diagnostic tests according to established protocols.

Participate in controlled substance counts at the start of each shift, adhering to safety procedures and standard precautions while responding to health emergencies within established guidelines.

Assist in medical or minor surgical procedures, implement nutritional and therapeutic diet plans, and document nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel.

Attend mandatory staff meetings and training sessions, demonstrating a commitment to ongoing professional development and adherence to policies and procedures in delivering high-quality patient care.

Qualifications & Requirements:
Education: Graduate from an accredited nursing program.

Experience: Preferably a minimum of one (1) year of clinical experience. Licenses/Certifications: Must possess and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse in the state of employment.

Must be able to obtain and maintain CPR certification.

Additional Information:

Shift differentials apply for evening and night shifts, as well as weekends.
