Overview

EmpowerMe Wellness is seeking a highly skilled and dedicated Certified Occupational Therapy Assistant (COTA) to join our team in a senior living community. As a COTA, you will play a vital role in providing physical therapy and wellness services to older adults, while also supporting care coordination efforts and contributing to program development and quality improvement.

Responsibilities
  • Provide physical therapy and wellness services to older adults under the supervision of a PT as part of a multidisciplinary team
  • Support care coordination efforts by conducting regular wellness checkpoint visits with patients, communicating with patients' providers and caregivers, and assisting patients with attending telehealth visits
  • Maintain professional practices and ethical standards in accordance with company-wide policies and clinical initiatives
  • Contribute to program development, quality improvement, and problem-solving
  • Assist with optimizing each patient's functional well-being and satisfaction
