Job Title: Advanced Practice Clinician in Transplant Cardiology

About the Role: Advocate Aurora Health is seeking an experienced Advanced Practice Clinician to join our Transplant Cardiology team. As a key member of our care team, you will provide high-quality patient care, collaborate with physicians and other healthcare professionals, and contribute to the development of our clinical programs.

Key Responsibilities:
  • Evaluate patients, diagnose conditions, and develop treatment plans in collaboration with physicians.
  • Perform physical examinations, obtain medical histories, and order diagnostic tests as needed.
  • Prescribe medications, therapies, and treatments according to established protocols and guidelines.
  • Counsel and educate patients on healthy behaviors, disease management, and self-care skills.
  • Coordinate patient care, including arranging ancillary services, pre-operative planning, and discharge planning.


Requirements:
  • Demonstrated expertise in cardiology, particularly in transplant cardiology, with experience in inpatient settings.
  • Current licensure as a Physician Assistant or Nurse Practitioner in Illinois (or Wisconsin) and certification from a recognized national organization.
  • Strong communication and interpersonal skills, with ability to work effectively in a multidisciplinary team environment.
