Lead Social Work Specialist

We are seeking a highly skilled and experienced Lead Social Work professional to join our team at Tenet Massachusetts. As a Lead Social Work, you will be responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination.
Key Responsibilities:- Conduct complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions.
- Integrate national standards for case management scope of services including transition management, care coordination, compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy.
- Lead a population of patients by service line and/or lead the team by being a resource to Tenet performance standards.
- Coordinate complex discharge planning and educate and mentor new social work staff.
- Master's of Social Work degree.
- 3 years of department experience or 4 years of acute hospital social work experience.
- LCSW or LMSW license.
