Job Summary

We are seeking a highly skilled and compassionate Lead Registered Nurse to join our ACT Team at Richmond Behavioral Health Authority (RBHA). As a key member of our team, you will be responsible for providing complex professional and advanced human support work in the care and treatment of patients in a public health program.

Key Responsibilities
  • Perform complex professional and advanced human support work in the care and treatment of patients in a public health program, planning, organizing and supervising the daily operation of medication management.
  • Administer medications, provide medical assessments, order medications, manage medication administration records, draw blood, manage urine screens, document activities, attend clinical staffing and training meetings, and related work as apparent or assigned.
  • Assess client needs, develop, implement and monitor service delivery and assistance plans, coordinate and monitor services with other agencies, counsel and assist clients, maintain records and files, prepare reports, and related work as apparent or assigned.
  • Lead oversight of all nursing duties on the ACT team and medication preparation to assure ACT medication management and room is in adherence with OSHA Standards.
  • Draw blood and perform urine drug screens, label specimens via computer printout, maintain thorough documentation of daily activities including nursing, treatment services, crisis intervention, case management, reimbursement, and productivity statistics.
  • Attend and participate in staff meetings as required, coordinate, schedule and ensure appropriate level of nursing services to clients to include monitoring, administration and documentation of injections, vena punctures and other nursing services.
  • Respond to medical and psychiatric emergency situations and calls emergency medical services for consumers and staff when warranted, provide a minimum of 40% of services in the field.
  • Perform essential personnel functions of weekend, holiday, after hours, and on-call work, report to ACT Supervisor in accordance with RBHA policy, which includes clinical and administrative problem solving, consultation, and training.
  • Communicate challenges and barriers in a solutions-focused way to ACT Supervisor gaps in care or concerns with medication oversight, specifically around adherence to timely administration of medications, labs and injections for ACT clients.
  • Assist/monitor ACT psychiatrist in tracking needed labs and coordination of field and office appointments under the direction of the RN Supervisor, maintain adequate nursing supplies.
  • Maintain MARS, oversee management of refills by promoting the ACT primary care workers when PCP visits need to be scheduled, troubleshoot barriers to medication delivery by collaborating with the ACT team, hospitals, pharmacies, RICH clinic and outside providers.
  • Coordinate client services and treatment with multiple service providers and agencies, refer and link clients to resources in the community for housing, dental clinics, NA and AA groups, food pantry, health clinics, primary care physicians, library for research or computer use, etc.
  • Provide care coordination services to clients as necessary, responsible for a small caseload of approximately five clients with high-risk medical needs and concurring psychiatric illness.
Requirements
  • Associate's degree with coursework in nursing and moderate experience working with individuals with mental illnesses, or equivalent combination of education and experience.
  • Thorough knowledge of the principles, standards, practices and techniques of professional nursing, thorough knowledge of the treatment, diagnostic, immunization and related medical services for medically ill patients.
  • Ability to accurately maintain patient medical records and prepare reports, ability to establish and maintain effective working relationships with clients, supervisors, directors, medical personnel, community partners, vendors, associates and the general public.
