We are seeking a skilled Clinical Social Worker to join our Hospice Medical Services team at Tufts Medicine Care at Home. As a key member of our interdisciplinary team, you will provide high-quality clinical social work services to patients and their families in various settings.
Key Responsibilities- Conduct psychosocial assessments and interventions in crisis situations, including issues of suicide, homicide, family violence, elder or child abuse.
- Develop and implement in-home care plans, exploring alternatives to in-home care and arranging for placement if necessary.
- Provide information, referral, and creative resource acquisition for individual and family needs, both within the hospital and in the community.
- Assist patients and families in understanding psychosocial factors impeding their maximal utilization of hospital and community resources.
- Provide medical social services to family members or caregivers on a short-term basis, removing impediments to effective treatment or recovery.
- Participate in outreach, case finding, and screening for high-risk issues impacting patient progress, participation, or ability to utilize resources.
- Assist with discharge planning to ensure continuity of care, consulting with social, governmental, and legal agencies as needed.
- Master's degree in Social Work (MSW) from an accredited program.
- Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Massachusetts.
- Minimum 2 years of experience in clinical social work, preferably in a home health care setting.
- Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
- Strong assessment and intervention skills, with ability to work in crisis situations.
