Wilson Healthcare and Rehabilitation Center is seeking a skilled MDS Coordinator to join our team. As a key member of our healthcare team, the MDS Coordinator will play a vital role in ensuring the highest quality of care for our residents.
Key Responsibilities- Stay up-to-date on current federal and state regulations, as well as professional standards, to ensure compliance and best practices.
- Collaborate with the MDS team to develop comprehensive care plans that accurately address the needs of our residents.
- Coordinate and manage the written plan of care for each resident, identifying their needs and goals.
- Assist nursing management in reviewing nurse's notes to ensure the care plan is being followed.
- Monitor resident status changes and ensure timely nursing or clinical involvement.
- Ensure MDS and support documentation are accurate and meet regulatory requirements.
- Perform regular audits to ensure documentation accuracy.
- Assist with the discharge process and complete necessary forms and reports.
- Maintain established infection control and standard precaution practices.
- Ensure confidentiality regarding sensitive health information of residents.
- Report hazardous conditions, damaged equipment, and supply issues to appropriate personnel.
- Maintain a comfortable, private, and dignified environment for residents.
- Communicate effectively with residents, visitors, families, peers, and supervisors.
- Attend and participate in departmental meetings and in-services as directed.
- Current State License as a Nurse
- CPR Certification
- MDS experience
- One year experience as a Nurse in a long-term care setting
- Knowledge and training in all aspects of the MDS process
