Job Description:

The Supported Community Living Specialist will work directly with individuals in their homes, providing hands-on support and guidance to help them achieve independence in daily living tasks.

Responsibilities:

  • Teach and train individuals in developing skills necessary for daily living, such as meal preparation, cleaning, and personal hygiene
  • Assist individuals with activities of daily living, including bathing, dressing, and grooming
  • Provide transportation and accompany individuals on outings
  • Orient new individuals to their home environment and assess their skills
  • Supervise individual's goals and complete data collection related to running those goals
  • Complete all records and documentation using proper forms as they relate to established procedures
  • Attend staff meetings and scheduled in-service training sessions

Requirements:

To be successful in this role, you must have a high school diploma or equivalent, a valid Iowa Chauffeur's license, and the ability to work independently with minimal supervision.

Physical Requirements:

This position requires frequent travel, lifting up to 50 pounds, and standing for extended periods. You must also be able to assist individuals in wheelchairs and provide physical care as needed.
