Job Title: Infusion Therapy Nurse

At Kaleida Health, we are seeking a highly skilled and compassionate Infusion Therapy Nurse to join our team. As an Infusion Therapy Nurse, you will play a critical role in providing high-quality patient care in the home setting.

Job Summary:

The Infusion Therapy Nurse will be responsible for coordinating the total Plan of Care for the IV patient, assessing, planning, implementing, and evaluating their care. You will work collaboratively with the IV Health Care team to maintain standards of professional nursing practice in the home care setting.

Key Responsibilities:
  • Coordinate the total Plan of Care for the IV patient, assessing, planning, implementing, and evaluating their care.
  • Provide leadership for the IV Health Care team, working cooperatively with other team members to maintain standards of professional nursing practice in the home care setting.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and high-quality care.
Requirements:
  • Associates Degree required.
  • 1-5 years of relevant experience.
  • Graduate of accredited School of Registered Nursing.
  • One (1) year medical/surgical experience as a RN.
  • One year infusion experience.
  • Current licensure to practice as a Registered Nurse in NYS.
  • Valid NYS drivers license.
