Position Summary

In this role, you will operate under limited supervision to deliver a high level of comprehensive healthcare to a designated patient demographic in partnership with medical personnel. You will adhere to established nursing and medical protocols, providing specific care instructions to nursing and support staff.
  • Conduct thorough medical assessments and evaluations, including diagnosis, treatment, follow-up, consultations, and health education tailored to the patient's age and circumstances.
  • Gather patient histories and formulate care plans, ensuring thoroughness and accuracy in documentation.
  • Request, obtain, and analyze relevant diagnostic tests.
  • Develop, order, and document suitable treatment plans, including medication prescriptions based on a detailed review of the patient's history and diagnostic findings.
  • Communicate care plans effectively with relevant healthcare providers, including collaborating physicians, primary care providers, nursing staff, and case managers.
  • Engage with patients and their families to explain care plans, disease processes, new treatment strategies, and medication regimens.
  • Deliver medical care to patients experiencing illness or injury, adhering to accepted standards of practice.
  • Provide health education to patients and families as necessary and appropriate.
  • Record medical findings and care provided in the patient's clinical documentation, including laboratory and imaging study results.
  • Educate patients on health maintenance practices and offer educational resources as needed.
  • Collaborate with physicians and other healthcare team members regarding patient health needs, outcomes, and the effectiveness of care plans.
  • Initiate contact with other healthcare providers as necessary.
  • Safeguard the privacy and security of all confidential health information, using and disclosing only the information essential for job performance.

Qualifications

  • Master's degree with completion of an accredited Nurse Practitioner Program is required.
  • Current state licensure as a Nurse Practitioner is mandatory.
  • Current state licensure as a Registered Nurse is required.
  • Current BLS certification or obtained prior to providing direct patient care is essential.
  • Experience is preferred but not mandatory.
  • Eligibility for DEA and CDS is required.

Knowledge, Skills, and Abilities

  • Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the patient population served, including understanding growth and development principles across the lifespan and assessing data relevant to the patient's specific needs.
  • Strong verbal and written communication skills are essential for effective interaction with patients, families, departmental units, and medical and nursing staff. Proven interpersonal skills are required.
  • Maintain current hospital safety and other mandatory training.
  • Updated immunizations as recommended for the practice area.

Patient Safety Commitment

Ensure patient safety in all job functions and through active participation in hospital, department, or unit safety initiatives.
  • Take action to address observed risks to patient safety.
  • Report adverse events and near misses to the appropriate management authority.
  • Identify potential risks in processes, procedures, and devices, and communicate these to relevant personnel.
