Behavioral Health Coordinator

As a Behavioral Health Care Coordinator with UnitedHealthcare in Conroe, Texas, you will play a vital role in simplifying healthcare experiences and creating healthier communities. This position is ideal for individuals passionate about ensuring quality care and removing barriers to it.
As part of our team, you will coordinate long-term care needs for patients in the local community, working closely with an elite and dynamic team. Your diverse knowledge and experience will be leveraged to make healthcare work better for our patients. The Behavioral Health Care Coordinator role is an essential element of an Integrated Care Model, where you will relay pertinent information about member needs and advocate for the best possible care available.
We are seeking candidates who possess a strong background in behavioral health, preferably with Medicaid Waiver experience, Case Management, and/or Care Coordination experience. If you have a proven track record of managing populations with complex medical or behavioral needs, we want to hear from you
About You:
We are looking for an individual who is able to assess, plan, and implement care strategies that cater to each patient's unique needs. You should be comfortable identifying and initiating referrals for social service programs, including financial, psychosocial, community, and state supportive services. As a single point of contact, you will manage the care plan throughout the continuum of care, communicating effectively with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
You will also advocate for patients and families as needed, ensuring their needs and choices are fully represented and supported by the healthcare team. Field-based work experience is highly valued in this role, especially if you have worked with electronic charting systems and have a background in managing complex populations.
