Key Responsibilities
  • Evaluate and assess patients to establish functional, measurable goals
  • Provide treatment according to a written plan of care to meet established goals
  • Plan for discharge and communicate findings and progress according to professional licensure/certification, scope of practice and departmental/discipline policies and procedures
  • Document interventions/care in the patient's medical record
  • Evaluate effectiveness of plan of care while also reassessing patient's functional status
  • Provide discharge planning information which includes patient/family education, follow-up, and referral to community resources
Requirements
  • Minimum: Entry level
  • Licensure, Registration, Certification: PT; BLS Certification within 14 days of hire
  • Special Skills: Basic skills such as keyboarding, using a mouse, and navigating sites
