About the Role: As a Registered Nurse (RN) Case Manager II, you will play a vital role in facilitating patient transitions from acute care settings to other appropriate levels of care. You will collaborate with physicians and payors to ensure effective use of resources and promote career growth through additional certifications and training.
Key Responsibilities:
- Facilitate patient transitions from acute care settings to other appropriate levels of care
- Collaborate with physicians and payors to ensure effective use of resources
- Develop and implement patient care plans that meet individual needs and goals
- Provide education and support to patients and their families on health-related topics
Requirements:
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred
- Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure is required
- Professional certification preferred
- Membership in a professional organization preferred
