Job Description

The Registered Nurse (RN) will be responsible for delivering high-quality patient care and services to a group of patients. This may involve visiting patients at their residences, which could include private homes, Skilled Nursing Facilities, or assisted living facilities. As a contributing member of the interdisciplinary team, the RN will provide expertise regarding psych-social assessments, interventions, and knowledge of community resources.

This role can be filled in either a Case Management, Admissions, or Resource capacity. We offer strong support from a patient care team, allowing you to work independently while maintaining close relationships with patients.

Qualifications

  • Current RN Licensure: Possess a current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
  • Team Collaboration: Work effectively within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Communication Skills: Develop strong communication skills to interact with patients, families, and healthcare professionals.
  • Time Management: Demonstrate excellent time management skills to manage visits, documentation, meetings, and urgent needs.
  • Electronic Medical Records: Be able to learn and utilize electronic medical records systems.
  • Reliable Transportation: Possess a valid driver's license, reliable transportation, and auto insurance to travel across the service area.
