Requirements
  • Current New York license as a LPN/LVN.
  • At least two years of recent experience as a LPN/LVN in a Long-Term Care - Skilled Nursing Facility setting.
  • Current BLS and/or ACLS and/or Specialty Certifications.
  • Two current clinical references.
  • Physical and TB records.
  • Current Tdap/Flu vaccinations.
  • Titers - MMR/Hep B/Varicella or vaccination records.
  • Ability to pass background/urine drug screening.
