About the Role

Human Development Services of Westchester, Inc is a leading organization dedicated to providing community-based direct-care services for vulnerable populations. We are seeking a qualified individual to join our team as a Healthcare Case Manager.

Job Summary

This is a unique opportunity to make a positive impact on people's lives. As a Healthcare Case Manager, you will be responsible for assisting clients living in the community to access necessary medical, behavioral health, and social services.

Key Responsibilities

The successful candidate will have strong written and verbal communication skills, with the ability to maintain accurate documentation and work effectively with clients, providers, and the healthcare community. Key responsibilities include:

  • Assessing clients' needs and developing personalized plans to meet their basic life needs.
  • Coordinating with healthcare providers to ensure seamless delivery of services.
  • Maintaining accurate records and reports, including client progress and outcomes.
Requirements

To be considered for this role, candidates must have a BA/BS in a human services field with a minimum of one year's experience in a behavioral health setting. Alternatively, an AA/AS in a human service field with two years of experience in a behavioral health setting may also be considered.

Strong knowledge of the behavioral characteristics and psychosocial needs of the target population is essential. Bi-lingual English and Spanish speaking skills are a plus. A valid driver's license and satisfactory driving record are required, as well as a Criminal History Background Check and fingerprinting.

Location

This position is based in Westchester County, NY, and requires travel to various locations within the county to meet with clients and partners.
