Job Description

Longevity Health is seeking an Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our team. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to our nursing home residents.

Key Responsibilities:

  • Provide collaborative primary care and services to a panel of Longevity Health members.
  • Develop and implement individualized plans of care for each member.
  • Collaborate with primary care teams and other healthcare professionals to ensure seamless care coordination.
  • Communicate effectively with members, families, and care teams to ensure understanding of member goals and needs.
  • Document member health status, functional status, and diagnoses accurately and thoroughly.

Requirements:

  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required.
  • DEA License required.
  • Must have an active national provider number (NPI) and an active Medicare provider number.
  • Valid state driver's license with a good driving record and proof of automobile insurance required.
