Job Summary

We are looking for a highly motivated and compassionate Physical Therapist to provide personalized rehabilitation therapy to patients in their homes and care facilities. As a member of our interdisciplinary care team, you will practice independently and conduct comprehensive assessments of patients to determine their needs and develop effective treatment plans.

Key Responsibilities
  • Deliver high-quality rehabilitation therapy to patients in their homes or care facilities
  • Conduct comprehensive assessments of patients to determine their needs and develop effective treatment plans
  • Practice independently as part of an interdisciplinary care team
  • Evaluate evolving patient needs and respond with plan of care adjustments
Requirements
  • Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern
  • Licensure to practice as a Physical Therapist in New York State
  • Valid driver's license required for all areas outside the borough of Manhattan
