At VNA Health at Home, we're seeking a dedicated RN to join our team of compassionate caregivers. As a Home Health RN, you'll have the opportunity to make a profound impact in your community, providing personalized care and support to our cherished patients.
Key Responsibilities:
- Perform comprehensive patient assessments and collaborate with our multidisciplinary team to achieve heartfelt outcomes and goals.
- Supervise your caseload and nurture the growth and development of LPNs and Home Health Aides.
- Facilitate care coordination and ensure seamless support for your patients.
Qualifications:
- Active RN License in the State of service.
- One (1+) Year of Clinical experience required; Hospital experience preferred, Home Health experience a plus.
- Current CPR Certification Required.
- Valid Driver's License.
