Your Role:
  • Provide direct, skilled nursing services to patients in accordance with their Plan of Care, under the direction of the RN Case Manager.
  • Build trusting, compassionate relationships with patients and their families, empowering them to make informed decisions about their care.
  • Collaborate with the care team to provide comfort, peace, and dignity to patients and their loved ones.
  • Empower caregivers by educating them on the care and support of their loved ones.
