Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of each patient
- Coordinate with healthcare providers and community resources to ensure seamless care transitions
- Provide skilled nursing services, including wound care, medication management, and education
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Document patient progress and outcomes in a timely and accurate manner
- Current RN licensure in the state of practice
- 1 year of acute care experience, preferably in a home health care setting
