Aveanna Healthcare is seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working closely with physicians and other healthcare professionals to develop and implement individualized care plans.
Responsibilities:
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Provide skilled nursing care to patients in their homes, including wound care, infusions, catheter care, and PICC line dressing changes.
- Educate patients and their families on disease management, medication adherence, and other health-related topics.
- Coordinate with primary caregivers to ensure optimal functioning levels for each patient.
- Document patient progress and communicate with healthcare teams to ensure seamless care transitions.
Requirements:
- Active RN license in the state of application.
- Valid CPR certification.
- 1-year RN experience in a healthcare setting.
- Basic understanding of Oasis.
