Baylor Scott & White Health is seeking a skilled Registered Nurse (RN) to join our Hospice team in a nursing role in Texas.
Job SummaryAs a Hospice Registered Nurse, you will coordinate patient care with a patient-centered approach and evaluate health responses. You will craft care plans and track patient progress.
Responsibilities- Develop and implement individualized care plans for patients with complex needs.
- Provide high-quality patient care, including administering medications, wound care, and other treatments.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Bachelor's degree in Nursing required.
- Two years of relevant work experience in Hospice or Home Health required.
- Must be a registered nurse. Obtain Basic Life Support certification within 30 days of hire.
