Job Title: LPN - Winnfield LA

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Autumn Leaves Nursing & Rehabilitation Center in Winnfield, LA.

Responsibilities:
  • Provide high-quality nursing care to residents in accordance with facility and nursing service policies and procedures.
  • Promote inter- and intradepartmental relationships to ensure seamless care delivery.
  • Administer medications accurately and safely, and be aware of pharmacologic and therapeutic effects.
  • Record nursing information on resident care plans and clinical records, including nursing notes, graphic sheets, medication, and treatment records.
  • Transcribe physician orders and observe, report, and record resident reactions to drugs, tests, treatments, and therapeutic measures.
  • Participate in staff development programs and assist with the orientation and training of nursing personnel.
  • Assume responsibility for residents assigned to care, including regular bed checks to ensure resident safety.
  • Report incidents of suspected resident abuse immediately.
  • Supervise nursing assistants working on shift and give and receive the nursing report upon reporting in and ending shift duty hours.
Requirements:
  • Must hold a current unencumbered license as a licensed practical nurse in this state.
  • Must be registered with the State Board of Examiners of Practical Nurses.
  • Must be a graduate of an accredited educational institution (school of nursing) or a graduate of a state-approved school of practical nursing.
  • On-the-job training as an integral part of the licensed-training program and at least one year of experience preferred.
