About the RoleThe Home Health Registered Nurse RN Case Manager is responsible for assessing the physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, and cognitive status of our patients, as well as their discharge planning needs. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Key Responsibilities- Coordinate and direct the care of a caseload of home patients, prioritizing their needs and adapting to changing circumstances.
- Provide comprehensive assessments, planning, implementation, and evaluation of care for each patient, ensuring their safety and well-being.
- Collaborate with our team to identify performance improvement initiatives and design programs to address needed changes.
- Monitor patient progress, adjusting care plans as needed to achieve optimal outcomes.
