We are seeking a highly skilled Recovery Support Coordinator to join our team at St. Vincent's Hospital Westchester. In this role, you will be responsible for providing comprehensive recovery planning and support services to individuals with severe mental illness.

About the Role

The ideal candidate will have a strong background in psychiatric rehabilitation and experience working with individuals who have multiple problems. You will be responsible for conducting initial and ongoing assessments, developing individualized recovery plans, and collaborating with other providers to ensure seamless care transitions.

Key Responsibilities

  • Conduct initial and ongoing assessments to identify individual needs and develop personalized recovery plans

  • Collaborate with other providers to ensure comprehensive care and smooth transitions

  • Provide person-centered interventions, including individual, group, and family sessions

  • Work with individuals who have multiple problems, promoting wellness and recovery

  • Maintain accurate documentation of client interactions and progress

  • Participate in psychiatric and medical emergencies as needed

Requirements

  • Master's degree in a relevant field (e.g., social work, psychology)

  • Professional licensure required

  • Prior experience working with individuals with severe mental illness and/or co-occurring disorders

  • Excellent communication and interpersonal skills

  • Ability to work in a fast-paced, dynamic environment

  • Proficient in Microsoft Office Suite and database navigation

  • Valid driver's license in good standing (preferred)
