Job Description:

Key Responsibilities:

  • Effectively screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
  • Develop appropriate plans of care following all regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Assess patients' progress and adjust treatments accordingly in clinical records per regulatory and clinical practice requirements.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers.
  • Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summaries of services per regulatory and clinical requirements.
  • Document all regulatory and clinically required information in the electronic documentation system.
  • Accurately record treatment minutes for services provided per all regulatory and clinical requirements.
  • Participate in required meetings including patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Engage in facility and company-required trainings, inservices, and conferences.
  • Obtain any necessary DME or supplies per regulatory and clinical practice requirements.
  • Supervise and support CFY or Student Speech-Language Pathologists.
  • Adhere to ASHA and state-required practice acts, code of conduct, and ethics during professional practice as a Speech-Language Pathologist.
