Nurse Specialist in Home Healthcare

About the Opportunity:
PRIORITY CARES HOME SERVICES is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team and provide personalized care to our client in Newark, NJ.
Job Overview:
In this role, you will be responsible for delivering high-quality care and support to our patient with developmental disabilities. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of our patient.
Key Responsibilities:
- Deliver skilled nursing care and support to our patient in accordance with established care plans and protocols.
- Administer medications and treatments as prescribed by the healthcare provider.
- Monitor and assess the patient's condition, including vital signs, respiratory status, and overall well-being.
- Assist the patient with activities of daily living, including bathing, grooming, and mobility support.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Communicate effectively with the patient, family members, and other healthcare professionals to ensure continuity of care.
- Maintain accurate and timely documentation of nursing assessments, interventions, and outcomes.
Requirements and Qualifications:
- Current licensure as a Licensed Practical Nurse (LPN) in the state of New Jersey.
- Minimum of 1 year of experience working as an LPN in a healthcare setting, preferably with individuals with developmental disabilities.
- Must have experience with ventilation or respiratory patients.
- Strong clinical skills and knowledge of nursing principles and practices.
- Ability to assess and respond effectively to changes in the patient's condition.
- Excellent communication and interpersonal skills.
- Compassionate and patient-centered approach to care.
- Ability to work collaboratively as part of a multidisciplinary team.
- Basic computer skills for documentation purposes.
Compensation and Benefits:
