Role Summary: The Advanced Care Practitioner plays a crucial role in addressing healthcare deficiencies for patients, aiming to reduce the necessity for repeated consultations. This independent position allows for the creation of a customizable schedule in collaboration with the Market Medical Director and care navigator, concentrating on lists generated by the healthcare system. The practitioner will deliver both in-person and virtual healthcare services, travel as needed, and manage clinical documentation and coding.

Key Responsibilities:

  • Design and oversee a flexible schedule based on identified healthcare gaps, in partnership with the Market Medical Director and care navigator.
  • Conduct patient visits to pinpoint and resolve healthcare deficiencies, ensuring they do not recur.
  • Provide virtual healthcare services on assigned days.
  • Travel to various healthcare facilities as required.
  • Prescribe and renew medications, including routine and new prescriptions based on identified healthcare needs.
  • Assess patients and perform in-office tests as necessary.
  • Engage in thorough clinical documentation and coding practices.
  • Participate in an orientation program followed by specialized training.
  • Contribute to the market's on-call schedule as needed.

Performance Metrics:

  • Annual performance bonus based on the number of patients seen and healthcare gaps addressed.

Qualifications:

  • At least two years of direct experience with coding and healthcare metrics.
  • Proven ability to identify and rectify healthcare deficiencies.
  • Experience in managing prescriptions effectively.
  • Strong skills in clinical documentation.
  • Willingness to travel and adapt to diverse work settings.
  • Capability to work autonomously and make informed clinical decisions.
  • Familiarity with telehealth technology.

Preferred Qualifications:

  • Previous experience in a role requiring extensive travel.
