Responsibilities
Follow established standards of nursing practices and implement facility policies and procedures. Interpret existing policies and procedures to other licensed staff, nursing assistants, residents, families, and physicians. Assume responsibility for compliance with Federal, State, and local regulations within the assigned unit.
Obtain report from nurse he/she is relieving and implement appropriate follow-up action as necessary. Provide report to nurse coming on duty, including sufficient information for follow-up action if necessary.
Conduct resident rounds, direct care for C.N.A.s and LPNs, and initiate corrective action and reporting issues/actions taken to nursing supervisor. Report changes in resident conditions to a physician, Supervisor, and responsible party promptly and take follow-up action as necessary. Identify resident problems and emergency situations, and initiate life-saving measures in the absence of a physician.
Supervise and evaluate all direct resident care and initiate changes or corrective action as necessary. Prepare and assign nursing assistants assignments and duties. Be able to explain all pertinent resident care.
Provide clinical supervision of certified nursing assistants providing direct resident care and give direction to licensed practical nurses. Assess the patient data collected by the licensed practical nurse.
Knowledgeably and competently perform basic nursing skills, including administration of medications (including IV, enteral) and treatments per the physician's order and facility policy. Follow facility policy and State Law in handling of all controlled drugs. Complete admission, transfer, and discharge of residents according to facility policy. Coordinate the admission/discharge/transfer with physicians and other departments as necessary.
Perform and document comprehensive assessment of each resident. Develop care plan based on these assessments. Participate in care conferences as requested. Document medications administered or refused, effectiveness of PRN medications, treatments given, wound care given. Document general nursing notes as required for to note patient condition/progress, change in status, unusual events, and as required by regulatory agencies, i.e., Medicare. Complete documentation related to the residents' admission, transfer, or discharge, i.e., W10, discharge plan. Ensure C.N.A.s have completed their patient documentation.
Counsel nursing assistants, licensed practical nurses, and recommend disciplinary action, when necessary, to the Shift Supervisor. Participate in the employee evaluation process by providing information on the clinical performance of certified nursing assistants to the Shift Supervisor.
Check emergency equipment and sign off on the emergency cart checklist sheet. Ability to order and receive medication delivery from pharmacy and check all drugs received. Complete 24-hour report sheet in a thorough manner. Complete accident/incident reports as necessary. Diligently observe infection control practices according to policy. Obtain continuing education relating to the resident population. Ability to observe all facility safety policies and procedures. Ability to follow Residents' Rights policies at all times.
Communication/Team Player/Attitude: Ability to consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers. Welcome new staff on the unit and act as a preceptor when assigned. Welcome the new resident and family in a positive and pleasant manner. Ability to come to work in a clean, neat uniform and consistently present an appropriate professional appearance. Ability to come to work as scheduled and consistently demonstrate dependability and punctuality. Ability to accept assigned duties in a cooperative manner. Perform other job-related duties as directed by the Nursing Supervisor and/or Nursing Administration.
Chaperone: Will be responsible to chaperone patients as necessary to appointments and be in the room with them for certain procedures, examinations, and treatments, i.e., pelvic examination; intravaginal physical therapy, examination treatments, or procedures of or involving the external genitalia (inclusive of urinary catheterization); examination, treatments, or procedures of the female patient's breast(s); rectal examinations or treatments, or procedures (inclusive of rectal tube insertion).
Qualifications
Baccalaureate degree in Nursing or Science preferred. At least one year of work experience in rehabilitative, psychiatric, or geriatric nursing preferred. A Registered Nurse licensed in the State of Connecticut. Excellent communication and interpersonal skills. Demonstrates the ability to maintain standards of care rendered at Grimes Center according to the State of Connecticut Public Health Code, the governing body of the facility, and the State and Federal agencies.
