Home Health Physical Therapist Opportunity

We're seeking a compassionate and experienced Physical Therapist to join our team in Kent County. As a Home Health Physical Therapist, you'll have the flexibility to work a minimum of 8 shifts per month, visiting patients in their homes and providing personalized care and therapy.

As a member of our team, you'll have the opportunity to work with a variety of patients, each with unique needs and goals. You'll develop and implement individualized care plans, perform diagnostic tests and measurements, and educate patients and their families on rehabilitative care and activities necessary to promote independent living.

We offer a premium rate differential and mileage reimbursement, as well as the opportunity to work with a dynamic and supportive team. If you're passionate about delivering patient-centered care and have a strong commitment to excellence, we encourage you to apply for this rewarding opportunity.

Requirements:

  • Active and current license to practice as a professional Physical Therapist (PT) in the state of DE.
  • Previous home health experience required.
  • Must commit to a 13-week assignment and a minimum of 8 shifts per month.
  • OASIS experience required; previous HCHB experience preferred.
  • Ability to travel throughout the region based on our census.
  • Ability to deliver patient care with compassion, excellence, and reliability.

Compensation:

  • Based on experience and type of visit - $200 - $212 per Start of Care visit
  • $80 – $85 per Routine visit
