- Provide all aspects of skilled nursing care and management of patients.
- Supervise the aide(s).
- Actively provide supervision of LPNs as it relates to patient care.
- Report admission information and changes in the patient's condition to the Patient Care Coordinator (PCC) and implement changes to the plan of care as discussed.
- Provide timely and appropriate patient/family education and support based on diagnosis and expected outcomes.
- Identify need for other internal and external patient care services, make appropriate referrals, and coordinate the overall care provided to the patient.
- Participate in quality assessment and improvement activities.
- Consistently provide responsible, professional guidance and support to the LPNs to assure continued clinical growth and excellence.
- Presents positive attitude through change, supports all organizational activities and processes directly benefitting the entire VNA Family of Services.
- Actively supports team members in every aspect of delivering care and daily responsibilities.
- Currently licensed as a Registered Nurse in the Commonwealth of PA.
- Must have dependable transportation with a valid driver's license.
- Independently visit patients in their homes.
